The UC Health Element: Claiming When Illness Limits Work

The Universal Credit LCWRA health element is £217.26 or £429.80 a month in 2026/27. The higher rate covers protected pre-April 2026 cases and qualifying severe, lifelong or end-of-life cases; most other conditions reported from 6 April 2026 use the lower rate.
In 2026/27 the Universal Credit health element (LCWRA) is either £217.26 or £429.80 per monthly assessment period. The rate depends mainly on when you reported the condition and whether the DWP decides that severe, lifelong or end-of-life rules apply. LCWRA also means you do not have to look for work or prepare for work.
This is not the same as telling Universal Credit that you are ill. You normally need to report the condition, provide fit notes and complete the Work Capability Assessment process before the DWP can decide that you have limited capability for work and work-related activity (LCWRA).
Step one: fit notes, immediately
Report the health condition through your Universal Credit account and provide a fit note if required. Continue reporting new fit notes before the previous one expires until the DWP tells you that you no longer need them. Keep copies and journal confirmations.
A fit note does not itself award LCWRA and does not automatically remove every work-related requirement. Your claimant commitment should take account of the condition while you wait; use the journal to ask what applies to you if it is unclear. GOV.UK lists limited exceptions where a fit note is not required, including some people moving from ESA without a break.
Step two: the Work Capability Assessment
If the condition affects your ability to work for longer than 28 days, you may need a Work Capability Assessment (WCA). The health questionnaire is now called WCA50; older advice may call it UC50. Return it by the deadline and explain what happens when you attempt each relevant activity, including safety, repetition and after-effects. The assessment may use your form and evidence and can include a telephone, video or in-person appointment.
There are three broad outcomes: fit for work; limited capability for work (LCW), which changes work-related requirements but usually adds no money for newer claims; or LCWRA, which removes work-search and work-preparation requirements and may add the health element. PIP and a WCA use different legal tests, so do not copy a PIP answer without adapting it to the activity the WCA asks about.

The date you reported the health condition can matter—not simply the later date of the LCWRA decision. If you told Universal Credit before 6 April 2026 and the assessment decision came later, official guidance says the higher amount can still apply. In a couple, only one LCWRA element is paid; if either partner qualifies for the higher amount, the higher amount is used.
The severe-conditions route is decided from medical evidence as part of the WCA. GOV.UK says the condition must mean you cannot work, last for life, not improve and be officially diagnosed. People nearing the end of life follow separate special rules and do not need a WCA.
The extra amount usually starts after three full assessment periods from when you began submitting medical evidence, although exceptions apply. It is not normally backdated merely to the day the decision is made. Ask for a statement breakdown if the start date or rate looks wrong.
PIP is separate — and claim both
The WCA decides your UC work-capability status; PIP is a separate benefit with a different test (see current PIP rates). You can qualify for both, either one, or neither. A PIP decision does not automatically award LCWRA, though relevant medical and functional evidence may help both claims.
Working a little, carefully
LCWRA does not ban work. GOV.UK says you can work if you feel able, and starting work alone does not trigger another assessment unless your health condition changes. Earnings still affect the Universal Credit calculation, but an LCW/LCWRA household can receive a work allowance before the 55% taper applies. If the decision or payment is wrong
Read both the decision letter and the next statement. Check the outcome, higher or lower rate, start assessment period and whether the element appears at all. If the DWP used the wrong facts, request a mandatory reconsideration, normally within one month of the decision date, and get independent benefits advice.

Do I need LCWRA before I can get PIP?+
No — they are independent. You can receive PIP while working full-time, or LCWRA without PIP. Claim each on its own merits; the evidence overlaps usefully, and a decision letter from one often strengthens the other’s file.
What happens while I wait for the WCA decision?+

Can they take LCWRA away at a review?+
